"Main recommendations

  1. ESGE/ESGAR recommend computed tomographic colonography (CTC) as the radiological examination of choice for the diagnosis of colorectal neoplasia. ESGE/ESGAR do not recommend barium enema in this setting (strong recommendation, high quality evidence).
  2. ESGE/ESGAR recommend CTC, preferably the same or next day, if colonoscopy is incomplete. Delay of CTC should be considered following endoscopic resection. In the case of obstructing colorectal cancer, preoperative contrast-enhanced CTC may also allow location or staging of malignant lesions (strong recommendation, moderate quality evidence). 
  3. When endoscopy is contraindicated or not possible, ESGE/ESGAR recommend CTC as an acceptable and equally sensitive alternative for patients with symptoms suggestive of colorectal cancer (strong recommendation, high quality evidence). 
  4. ESGE/ESGAR recommend referral for endoscopic polypectomy in patients with at least one polyp 6 mm in diameter detected at CTC. CTC surveillance may be clinically considered if patients do not undergo polypectomy (strong recommendation, moderate quality evidence). 
  5. ESGE/ESGAR do not recommend CTC as a primary test for population screening or in individuals with a positive first-degree family history of colorectal cancer (CRC). However, it may be proposed as a CRC screening test on an individual basis providing the screenee is adequately informed about test characteristics, benefits, and risks (weak recommendation, moderate quality evidence)."

Del abstract:

"Proposed guidelines are: follow-up is not advised in patients with isolated TML in the absence of risk factors (see Key Points below); annual ultrasound (US) is advised for patients with risk factors, up to the age of 55; if TML is found with a testicular mass, urgent referral to a specialist centre is advised.

Consensus opinion of the scrotal subcommittee of the ESUR is that the presence of TML alone in the absence of other risk factors is not an indication for regular scrotal US, further US screening or biopsy. US is recommended in the follow-up of patients at risk, where risk factors other than microlithiasis are present.

Key Points
• Follow up advised only in patients with TML and additional risk factors.
• Annual US advised for patients with risk factors up to age 55.
• If TML is found with testicular mass, urgent specialist referral advised.
• Risk factors – personal/ family history of GCT, maldescent, orchidopexy, testicular atrophy."

Avances contra el cáncer 2015

Desde hace diez años la American Society of Clinical Oncology publica en enero el resumen de los avances del último año en el diagnóstico, la prevención, el screening, el tratamiento, la investigación básica y el seguimiento del cáncer.

Muy esquemático y resumido, no es para especialistas (¡¡la mayoría no son sobre fármacos!!). Y como cumplen diez años también resumen los avances de la década.

... es un ensayo clínico: Multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands, evaluando el tratamiento intraarterial de las oclusiones agudas proximales de la circulación anterior (los resultados sugieren que mejora los resultados funcionales). Se publicó en el New England el 1 de enero

* Es un ensayo pragmático, para el que no esté familiarizado con el término:

Patsopoulos 2012: "The term explanatory was used to describe trials that aim to evaluate the efficacy of an intervention in a well-defined and controlled setting, whereas the term pragmatic was used for trials designed to test the effectiveness of the intervention in a broad routine clinical practice. The explanatory trial is the best design to explore if and how an intervention works, and the whole experiment is designed in order to control for all known biases and confounders, so that the intervention's effect is maximized. Usually the intervention under examination is compared with a placebo or with another active treatment. The pragmatic trial, on the other hand, is designed to test interventions in the full spectrum of everyday clinical settings in order to maximize applicability and generalizability. The research question under investigation is whether an intervention actually works in real life. The intervention is evaluated against other ones (established or not) of the same or different class, in routine practice settings. Pragmatic trials measure a wide spectrum of outcomes, mostly patient-centered, whereas explanatory trials focus on measurable symptoms or markers (clinical or biological). "

Los segmentos hepáticos se escriben con números arábigos y no con números romanos.


Para enumerar los segmentos hepáticos utilizamos el sistema de Bismuth porque la práctica quirúrgica se correlaciona muy bien con la terminología radiológica. Este sistema utiliza números arábigos para identificar cada segmento (1, 2, 3, 4, 5, 6, 7 y 8), aunque los radiólogos generalmente utilizamos (de forma errónea) números romanos en nuestros informes radiológicos.
